Just a suggestion for the On-Demand environment ... I would request clients be able to "clone" communication forms. I had to create over 25 forms with layouts, custom html messages, and responses. Would have been a big time saver to hit clone... call it something else and away I go.

We actually have a story on our engineering backlog for copying/cloning a form (but it's not yet scheduled for a sprint). I'll add a link from that story to your request. BTW, on the input end, are you using scenarios to populate property values? Seems like something that might really work for you.
I cannot actually add a reply in here. So just typing right here. This particular instance is an integration with HP Service Manager providing the inputs. However the client was particular about which criteria in tickets get sent in different devices. The solution we came up with was multiple forms in the plan and using code inside HPSM to shift the flow to those different forms.
